By agreeing, I understand and accept the services outlined and completed. I acknowledge that Touch of Paradise Designs, Inc., its employees, or representatives shall not be liable for any loss or damage to existing pumps and aquatic life. We take great pride in our service and treat each feature as if it were our own, handling each fish, plant, and mechanical product with care. Occasionally, fish may become stressed during the process and might not survive. By signing below, I hereby release and waive any claims against Touch of Paradise Designs, Inc., regarding any loss or damage to mechanical and aquatic life during or after the cleaning process.

This service is invoiced as follows: $150.00 Service Fee + $95.00 per man-hour of labor + Water treatments and additional supplies used or replaced. (A 4% service fee will be added to the invoice if paying by credit card.)

I understand an invoice will be sent upon completion of service, and I agree to pay it upon receipt.
